Centegra Health & Wellness Network (CHWN) providers specialize in preventive care to keep their patients healthy. Our goals are to:

  • Keep you out of the hospital

We recommend our patients take advantage of the screenings and preventive services available to the members of CHWN.

The Centegra Health & Wellness Network currently tracks the following measures:

Adult Wellness & Prevention

Adults who are members of CHWN are encouraged to schedule regular check-ups with their primary care providers. These appointments help patients establish communication with their providers when they’re well so they can communicate comfortably if they become ill.

The primary care providers who are part of CHWN help their patients stay well by monitoring Body Mass Index (BMI), blood pressure, cholesterol levels and other health information. They also encourage patients to talk about alcohol use and smoking to gain the best understanding of their overall health.

+ Cardiovascular Disease Prevention and Management

CHWN takes a unique approach to cardiovascular disease prevention and management. Members receive information about healthy blood pressure, cholesterol management and quitting smoking. If a person has cardiovascular disease, CHWN doctors and providers develop a program to prevent worsening of the disease and to improve his or her health.

CHWN providers help their patients manage diabetes and prevent other illnesses that often occur at the same time. Regular physician visits and lab work help patients improve their health. Screenings for kidney and eye disease help providers monitor the progression of diabetes. Our providers also teach people with diabetes about:

+ Asthma Action Plans

CHWN’s members who have asthma receive close monitoring and education about their conditions and medications. Members receive personalized Asthma Action Plans that detail how to control asthma, encourage people to stop smoking and provide information about special programs offered through Centegra Health System.
+ Appropriate Follow-up Care

CHWN encourages patients to schedule follow-up care after hospitalizations and visits to Centegra Health System’s Emergency Departments or Centegra Immediate Care locations. These appointments help providers make sure their patients are recovering according to plan, whether it’s from a broken bone, inpatient surgery, the birth of a baby or from the need for mental health services.

To promote cost savings for members, CHWN providers encourage the use of generic medications when possible.


Pediatric Wellness & Prevention

The pediatricians and family practitioners who are part of the Centegra Health and Wellness Network support parents from the time their babies are born until they’re ready for adult care. One of the best ways our providers help families stay healthy is through well-child appointments and preventive care.
Our providers answer questions about baby milestones and support parents through every stage of their children’s development. They help families prevent illness through vaccinations and by encouraging exercise and healthy nutrition for family members of all ages. Whether your child has just had a birthday or needs to be seen right away for illness, Centegra Health and Wellness Network providers have appointments available to address your needs.

+ Childhood Immunizations

CHWN providers understand that childhood immunizations are the best way to prevent and control serious diseases. Our doctors and providers recommend that all children receive the vaccinations that are recommended by the Centers for Disease Control and Prevention. For children who are younger than two years of age, these include DTP, Polio, MMR, HIB, Hepatitis B, Hepatitis A, Chicken Pox, influenza, Pneumococcal and Rotavirus. CHWN uses reminders and outreach programs to encourage its members to support childhood immunizations.
+ Special care for children who are above the recommended weight

CHWN is dedicated to preventing disease in children. If a pediatric patient has a Body Mass Index (BMI) above the normal range, he or she is eligible for additional screening and support that includes counseling for physical activity and counseling for nutrition.
